Dear Music Education Student:
 
We are writing to invite you to participate in a research study, Pre-Service Music Educators’ Perceptions of Functional Piano Skills. The purpose of this study is to document pre-service music teachers’ perceptions related to acquisition and use of functional piano skills. Documentation of these perceptions will inform music teacher education curriculum development and professional development for in-service music teachers.
 
Please take five to ten minutes to complete the linked survey. Responses will be anonymous.
 
Participation in this study is voluntary. By accessing the survey on Qualtrics and completing it, you are providing informed consent. An Informed Consent Form, approved by Institutional Review Boards at the University of Delaware and James Madison University, may be found at the end of this email.

INFORMED CONSENT TO PARTICIPATE IN RESEARCH
Title of Project: Pre-Service Music Educators’ Perceptions of Functional Piano Skills

You are being invited to participate in a research study. This consent form tells you about the study including its purpose, what you will be asked to do if you decide to take part, and the risks and benefits of being in the study. Please read the information below and ask us any questions you may have before you decide whether or not you agree to participate.
WHAT IS THE PURPOSE OF THIS STUDY?
The purpose of this study is to document pre-service music teachers’ perceptions related to acquisition and use of functional piano skills. You are being asked to participate because you have been identified as a music education student at a National Association of Schools of Music (NASM)-accredited institution. If you are not a student at a NASM-accredited institution, then you are not eligible to participate in this study.
WHAT WILL YOU BE ASKED TO DO?   
As part of this study you will be asked to complete an online survey, asking you to rate your level of agreement with several statements related to functional piano skills and techniques.
WHAT ARE THE POSSIBLE RISKS AND DISCOMFORTS?
We do not expect participation in this study to expose you to any risks different from those you would encounter in daily life.
WHAT ARE THE POTENTIAL BENEFITS?
Participating in this study may cause you to reflect on your personal musicianship and the role of functional piano skills in your future work as music educators. Findings from this study will inform future curriculum development for pre-service music educators, professional development for in-service music educators, and our profession’s understanding of the role of functional piano skills in music teacher preparation and practice.
HOW WILL CONFIDENTIALITY BE MAINTAINED? WHO MAY KNOW THAT YOU PARTICIPATED IN THIS RESEARCH?
The survey is anonymous. If you participate, we will be unable to identify your name. Results from this study will be reported in aggregate, summary form only. We will store results of this research on password-protected files, on a computer stored in a locked office for three years following completion of this research.
WILL THERE BE ANY COSTS TO YOU FOR PARTICIPATING IN THIS RESEARCH?
There are no costs associated with participating in this study.
WILL YOU RECEIVE ANY COMPENSATION FOR PARTICIPATION?                                  
There is no compensation for participating in this study.
DO YOU HAVE TO TAKE PART IN THIS STUDY?
Taking part in this research study is entirely voluntary. You do not have to participate in this research. If you choose to take part, you have the right to stop at any time. If you decide not to participate or if you decide to stop taking part in the research at a later date, there will be no penalty or loss of benefits to which you are otherwise entitled. Your decision to stop participation, or not to participate, will not influence current or future relationships with the University of Delaware or James Madison University.
